Admission Insight

The couples match lets two applicants link their rank lists so they are matched to a pair of programs. They either both get a listed combination or go unmatched together.

For two international applicants this compounds an already narrow search. Each list is already filtered by visa sponsorship, graduation year, and competitiveness. Coupling requires those filtered lists to overlap geographically.

It is entirely doable. It requires planning that starts earlier than most couples begin.

What you are actually optimising

The list you submit is a list of pairs. Each line names a program for each of you, and the algorithm works through them in order.

The critical implication is that the number of viable pairs, not the number of programs either of you likes, determines your safety. Two applicants each holding a long list of interviews can still construct very few workable pairs if those interviews are scattered across the country.

This is why geography does more work in a couples match than anything else, and why it needs to be a deliberate strategy rather than an outcome.

The international-specific compounding

Run the arithmetic honestly. Start with all programs in your specialty. Remove those that do not sponsor your route. Remove those whose graduation year filter excludes you. Remove those where your scores put you outside their range. Now do the same for your partner. Now keep only the areas where both filtered sets have programs.

For two international applicants in competitive specialties, that final set can be small. Discovering how small in September is a bad time. Discovering it in spring gives you options: broadening geography, one partner reconsidering specialty, planning for a year apart, or not coupling at all.

Strategy that helps

Do the filtering exercise together, early. One shared spreadsheet, one row per city, with each partner’s viable programs. The empty rows are your answer about geography.

Concentrate applications geographically. Applying to many programs in fewer metropolitan areas usually beats scattering across the country. Large cities with multiple teaching hospitals are structurally friendlier to couples.

Coordinate interview scheduling. An interview that does not pair with anything your partner has is worth less than it appears.

Be realistic about asymmetry. If one partner is applying to a much more competitive specialty, the couple’s outcome is largely governed by that partner’s list.

Include pairs you would tolerate, not only the ones you want, including combinations involving some separation if you both accept that.

The conversation to have first

What happens if only one of us can match? Coupling binds your outcomes. There are circumstances where matching separately gives each of you a better individual chance, at the cost of possibly being apart. Making that decision in advance is much better than in February.

How much distance is acceptable, and for how long? For international couples, a year in different countries with a visa constraining travel is a real possibility and should be discussed explicitly.

FAQ

Does couples matching hurt our individual chances?

It changes them rather than uniformly reducing them. You are matched on pairs, so a program that would have taken you individually may not appear in a viable pair. Whether that costs you depends on how well your filtered lists overlap geographically.

What if one of us is a US graduate and the other is an IMG?

This is common, and the visa filter then applies to only one list. That partner’s constraints will generally drive the geography, and building the shared list from their filtered set outward is the practical approach.

Should we tell programs we are couples matching?

Many couples mention it during interviews and programs are used to it. Check the current rules on what may be discussed and when, and keep it factual rather than turning it into a negotiation.
